Change Requests

To submit a change request, locate your church on the map, click on its pin, and then click the "Request Changes" link. If your church is not already on the map, then please read below for the information you must provide when submitting a request.

IMPORTANT: Before submitting a request, please read the FAQ for answers to common questions.

To submit a correction to existing info:

1) Locate the church on the map.
2) Click on the CFM Logo for the church to view info.
3) In the popup box, click the "Request Changes" link.

You can also click here to submit a request, but please be sure to provide a valid email address in case we need more info.

Address Changes

The CFM Directory stores two different address: the mailing address and the church address.When submitting address changes, please indicate if you are requesting changes to the MAILING address or CHURCH address.

Information Required for New Pastors

Requests for new pastors should include the following info:

  • First name
  • Last name
  • Wife's name
  • Complete CHURCH address
    • This address will appear on the map.
    • If there is no church address, then please say so in your request.
  • Complete MAILING address
  • Email address
  • Phone number and type (e.g. Mobile, Church, Home)
  • Mother church pastor's name and city.

Information Required for Pastoral Changes

If the new pastor is not already in the CFM Directory, you must provide all of the information required for new pastors. Otherwise, please include the First Name, Last Name, Wife's Name, and a valid email address or mobile number where we can email or text you if we require more information.
